Overhaul README + add GitHub Release workflow + npm wrapper

README: Complete rewrite reflecting 11 chains, 108 protocols,
22 commands. Includes live output examples with table formatting.

Release workflow: Triggered on v* tags, builds macOS arm64 +
Linux x86_64, creates GitHub Release with downloadable binaries.

npm: @hypurrquant/defi-cli wrapper with postinstall binary download
and ASCII branding.
