docs: add 28 practitioner module wiki articles (end-user + admin) End-user set (16): module overview, register doctor/consultant/staff, search & list, signatures, institution link, deactivate & bulk delete, bulk upload, OPD payments (per-shift + all-due), misc fees, inward professional/surgery, WHT, working times, communication history. Admin set (12): module configuration overview, specialities and doctor specialities catalogs, staff categories/designations/grades, link staff to system user, bulk upload format, WHT configuration, OPD payment behaviour, channel-doctor SMS notifications, privileges and access, retirement vs hard delete, audit trail. Existing thin stubs (Add-a-Doctor, Add-a-Consultant, Add-Doctor-Specialities, OPD-Doctors-Pay, OPD-Doctors-Mark-In, OPD-Doctors-Mark-Out) replaced with one-line redirects to the new articles so inbound links continue to work.
