Fix Radio Buttons Reverting to "No" in Prelim and Accessibility Sections

Radio buttons on Prelim-Operations, Prelim-Additional, and Accessibility reset back to No when navigating back to page, not sure the best way to fix this.

This bug was introduced and detailed in #123
