feat: add GraphOrchestrator — thin layer over Store with embedding, mirror, events

Orchestrator wraps Store + ProjectScopedStore, adding:
- Async embedding generation before create/update/search
- File mirror writes (.notes/.tasks/.skills/.epics)
- Event emission for all mutations
- Attachment file I/O + metadata registration
