Duplicate entries in menu when click over name

Describe the bug
When you click over you name customusermenuitems are repeated twice.

To Reproduce
Steps to reproduce the behavior:

1. Login
2. Click over your name
3. You will see repeated options

Expected behavior
We should be able to deactivate customusermenuitems that comes from general config

If I remove from the default core and a user uses another theme it does not appear in the other theme. So, in order to fix adaptable, I ruin boost.

Most of the items not defined by 'customusermenuitems' can be set on the 'Header user' theme settings tab.
