Q: how is unread status synchronized?

How is the read status/label tracked? Is it the unread label that gmail knows of? Any idea how this relates to (neo)mutt MUA read/unread/new statuses?

Asking this as I want to be able to toggle read/unread status in mutt, and have these changes be reflected in gmail.

This would also offer guidance in how to properly mark some messages automatically as read with notmuch + afew, e.g. something like notmuch tag +unread -- tag:lists AND -tag:unread

Ah interesting, so it also handles the maildir flags!? So effectively both maildir's new flag and gmail's unread label are effectively synced up?

GMail doesn't know about the maildir flags, so that's only to keep unread and maildir in sync for mail clients locally. I don't remember if maildir to unread is synced, or only the other way.
