Consider single "name" for both jitsi and etherpad#166

ATM Etherpad needs to have name entered separately from Jitsi. In the future it might be worth a simple dialogue + cookie (or "auth via github or google") to store visitor's name upon initial visit to be "fed" into both systems for consistent presence/annotation
