Maintainer question: which evidence failure would you distrust most?

I am trying to decide which failure modes deserve the clearest remediation in PatchGate. Please reply with A, B, C, or D, and add one sentence about why.\n\nA. A successful check from the wrong commit\nB. An approval that became stale after a new push\nC. Incomplete CODEOWNERS or permission data\nD. A policy change that tries to relax its own pull request\n\nThis is not a request for endorsements. I want to turn the most confusing case into a small fixture and use the discussion to check whether the result is understandable to someone who did not design the evaluator.
