hide "Additional Notes" field

After the Description field has been added, let's hide the Additional Notes field it was primarily designed to replace.

However, I think the boolean fields this column draws from may definitely still find eventual life within the tool, maybe as the toggles that we discussed a bit during the hacking weekend. I think they merit a thoughtful reconsideration, though, in terms of what should be included, etc.
