Standardise on soft-wrapping Markdown lines

• Re-wrap all the Markdown content to be soft wrapped.
• Add a note about this in an appropriate place so that it's done going forward.
• In a follow-up PR add the commit to Git ignored commits so it gets skipped in GitHub blames.
