If you're in combat, and you have potions you could have given to an npc on your team, but instead you just let them die, this should make them like you less. If you resurrect them quickly, your relationship is restored. But the longer you take to res them, the less of the relationship will be restored because you obviously don't value them. For balance, perhaps healing an npc when they're under 50 percent hp gives a small relationship boost. But maybe cap it so the boost only happens if you're relationship is under a certain level; just healing someone should probably not make them fall in love with me. But it would make them friendly/trusting.
