/v2/characters/:id/quests does not show all completed quests.
According to own tests and an info message over at https://gw2efficiency.com/account/character/:id?filter.navigation=story-journal&filter.buildTab=1&filter.equipmentTab=1 this problem occurs, if the episode was played more than once with that character.
To reproduce play an episode a second time and check the API endpoint.
