Users can create multiple instances, crashing the course

Users can create multiple instances of block_stash, which makes the course unusable.

Steps to reproduce:
* Create a new course
* Create a block_stash instance on the course page
* Go to the course edit page
* Create a new block_stash instance there
* Confirm the course is broken now
