We would like to have a way for referrals to come from our website. A "Get Help" button or link that would go to an online form with a submit button that would send that referral to our info@pc2online.org email. Here is a link to one of our community partners website, ODMF, that is an excellent example of what we are looking for (note the difference in the form from a self-referral and a professional referral).
