Create Category & Category List View

The Category List page, which also allows creating categories needs some clean-up of the design, as it is not intuitive and there are some unnecessary symbols and functions.

1. Remove the panel open/close feature
2. Remove the bullets on the left hand side listing the categories
3. Remove "Under Construction" Messaging
4. Allow user to clearly see a list of all existing categories, showing the category levels (sub-cats within parent-cats)
5. Allow a user to create a root category
6. Allow a user to create a sub-category, and make is clear that they are creating it within a root category
7. Don't show the "Create root category" form until a user makes an action to "create category". It's confusing that that form is the first thing you see on the page, instead it should show the category listing
