How do you add a new keybind?

I can't really find anywhere that describes how to add one. I see the plus button but when I click on it nothing happens. there are a few extra key binds that have so many arrows and extra like keys, but idk where they came from. I also can't remove them. Any idea where I can find how this works?

idk how to do the game log thing but here's a screen shot.

The ones that couldn't fit in the image are Sodium and Carpet pvp
