Doctor declaration Malaria processing fails if missing elements

Bug Description

It seems that there are further issues with Malaria regarding optional elements.

NullPointerException will be thrown if Malaria doctor declaration xml does not contain the following elements:

• profOccupation
• prophylaxisTreatment
• purposeOfTravel
• diseaseTransmission

Reproduction Steps

1. Create a doctor declaration xml with MALA section empty
2. Submit the doctor declaration
3. Mapping fails due to NPE

Expected Behavior

The xml should be converted to a message in the message directory.
