Incoming DMOD support in fluidsynth

Hi everyone,
After revising my DMOD proposal I contacted fluidsynth devs again, and Tom (the main maintainer) was very positive about the new version and agreed on making a PR.

I've just created it now and I'm fairly certain that we'll be getting full DMOD support in fluidsynth!
