set_table_style 什麼都沒改卻回 success「No style changes applied」（先 storeDocument 再回報）——#201 stub 家族的另一員

問題

Sources/CheWordMCP/Server.swift:8089（head a39253d）：set_table_style 在沒有任何樣式變更時先呼叫 storeDocument、再回傳「No style changes applied」——isError 未設、也不是 Error: 字串，所以 #202 的轉換與 sweep 都看不到它。這是 #201 / #172 那一族（回報成功但東西不在）的變體：不是謊稱做了，而是把「什麼都沒做」包成 success，且還把文件標成 dirty。

要做的事

• 沒有任何變更時：要嘛 throw ToolRefusal("set_table_style: no style properties given")（與 拒絕／錯誤以 return "Error: …" 字串回傳時 MCP isError 未設——協定層仍是成功（repo-wide 慣例） #202 慣例一致），要嘛回一個明確的 no-op 結果且不 storeDocument；二選一，不要現在這種「存了、但說沒改」。
• 補一個 protocol 測試（直接打 handleToolCall）。
• 順手掃同形狀：先 storeDocument 再回「No … applied / nothing to do」的 handler。

同家族再加一員（#202 R2 in-scope 修正時撞到）：splice_paragraph_omath_from_source 在來源段落沒有 OMath 時回「Spliced 0 OMath block(s) into target_paragraph_index=N」的 success，而且連 target 都不驗——target_paragraph_index: 99（文件只有 2 段）照樣回成功。文件一個位元組沒改、client 拿到 isError 未設的成功。處置同本票：來源無 OMath 應 throw ToolRefusal（或明確 no-op 結果且不 storeDocument），target 越界應在 0-block 路徑也驗。來源：#213 (comment)
