Issue Type: Lack of an ETL convention for a CDM table and/or field

Issue Name: Updating OMOP Observation Table to represent Gender Identity (GI) and Personal Pronouns as standard concepts

Description: There are standard concepts in HL7 and ONC to represent capture of Gender Identity and Personal Pronouns in clinical datasets. The GI and pronoun data elements should be stored as Observations as, unlike recorded birth sex, these elements may change over time as a person realizes their own identity through medical and/or surgical treatment. These elements also may not align with recorded birthsex and therefore are related closely to Observation Period for the purposes of observational research design.

Suggested solution: Represent GI and Pronouns as standard vocabulary using value sets published for HL7 FHIR and ONC USCDI v5 for Gender Identity [https://terminology.hl7.org/6.0.2/ValueSet-gender-identity.html] and LOINC Value Set for Pronouns (LOINC 90778-2)
