Show a modal overlaying the outermost body (cascading iframes)

We have a special scenario where we have cascading iframes. We were able to 
find the outermost body and supplied it to attachTo. But simpelmodal itself 
uses the window and document objects to determine the correct position. 

Attached is a patched 1.4.4 where we search for the outermost window and use 
that for the calculations. 

Maybe someone could incorporate it in this wonderfull library. We think that 
the changes do not harm.
