Add Related Reason where the user will select from occupational, medical or socioeconomic history

Add Related Reason where the user will select from occupational, medical or socioeconomic history or case plan. Place the new field to select it based on what exists from entries in occupational history, medical history or socioeconomic history. It should be formed so they can know that its from say occupational history, education and the title of the education to select from and so forth.

For each of those areas above say education, it should also include a link to the referral information in cases since its now related to a referral from the referral module

Add "Related Related Reason" right above referral reason. There could be multiple related they can select from the occupational history, medical history, socioeconomic history or case plan

It should be a link so they can go back to the history for which prompted the referral
