Update Summary Report to provide to investigators

Modifications to the Summary report based on feedback from the DICOM Subgroup

• Create separate Summary report for each Collection
• Remove the Collection from the column "Collection+BlindedSiteIDs+EventIDs with Issue"
• Include Profile as a column in the report
• Sort by Issue, then Profile
• Create a separate PHI report for each Collection that contains all the PHI issues. This does not need to have the Profile column.
