fix: Improve NPM publishing workflow with comprehensive debugging

- Remove unnecessary scope configuration from setup-node
- Add comprehensive NPM authentication testing
- Add version conflict checking before publishing
- Add npm ping and package existence checks
- This should help identify the exact cause of publishing failures
