Disappearing files during indexing halt the indexing process

Files are gathered at the beginning and might disappear later. This shouldn't halt the process. Just skip those files

Fixed in 1f5adbf. Files are gathered during the scanning phase and loaded later in batches. If a file disappears between these two phases, load_file() now checks for existence upfront and returns an empty document list with a warning log, rather than letting downstream libraries (llama-index, python-docx, etc.) raise an exception. This covers all file types and both the parallel and sequential loading paths.
